PATIENTS' BILL OF RIGHTS
An individual admitted to this clinic is entitled to the following rights:
- An individual plan of treatment in which you have participated.
- An explanation of the services provided in the treatment plan.
- Voluntary participation in treatment, unless you are court-ordered here, or you present a risk of physical harm to yourself or others.
- To object to or disagree with any portion of your treatment plan.
- To have all your records protected by confidentiality.
- To have access to your records, unless there is a clinical reason why you should not.
- To receive clinically appropriate care and to be treated humanely and with dignity.
- To receive services free of discrimination.
- To be treated in a way which acknowledges and respects your cultural environment.
- To receive the maximum amount of privacy consistent with the effective delivery of service.
- To be free from abuse and mistreatment by employees of this clinic.
- To be informed of the clinic's Grievance Policy.
